Becoming a Patient

The first step in becoming a patient is to set up an initial office visit with Dr. Hesselink. At your first visit, please bring the following applicable medical records, if you have access to these:

  • Relevant Lab Work
  • Written Reports of Imaging (CT, PET/CT, MRI, ultrasound, thermography, etc.)
  • Pathology Reports
  • Surgical Reports
  • Written Consultation Notes
  • Flow Sheets of past therapies or treatments you may have received

We will make copies of all the documentation you bring in, so you may keep the originals. Initial paperwork can be downloaded here. If you would like to fill these forms out ahead of time, please do so. If not, we will provide forms for you to kindly fill out at the time of your visit. This is not homework!

Where to Stay

The American Cancer Society has partnered with several hotel chains, including Extended Stay America, to provide low-cost or free overnight lodging for people traveling to receive cancer treatment. To inquire about availability and book a room call The American Cancer Society: 1-800-227-2345. During busier seasons, availability is limited. Please call the hotels below directly to book. They still offer reasonable nightly and weekly rates.

The Chicago suburbs offer an abundance of grocery stores and restaurants with healthy food choices. The following grocery stores are all within a three mile proximity of the clinic:

Extensive salad bar, hot bar, and prepared deli foods. Also a great selection of produce, clearly labelled either organic or conventionally grown. Located in the Country Line Square just down the street from the clinic, Brookhaven Marketplace also has clearly labelled organic vs. conventionally grown produce. They also have an extensive deli and prepared foods section for easy-to-prepare meals, and a variety of European brand items including an astounding array of sauerkrauts.


 

Getting Around

For travelers arriving by air, we are closest to Chicago Midway International Airport. Chicago O’Hare International Airport is also an easily accessible airport, though busier than Midway.

Transportation to and from each airport is best arranged ahead of time. Taxi 303 provides set rates between Burr Ridge and either airport:
