The first step in becoming a patient is to set up an initial office visit with Dr. Hesselink. At your first visit, please bring the following applicable medical records, if you have access to these:
- Relevant Lab Work
- Written Reports of Imaging (CT, PET/CT, MRI, ultrasound, thermography, etc.)
- Pathology Reports
- Surgical Reports
- Written Consultation Notes
- Flow Sheets of past therapies or treatments you may have received
We will make copies of all the documentation you bring in, so you may keep the originals. Initial paperwork can be downloaded here. If you would like to fill these forms out ahead of time, please do so. If not, we will provide forms for you to kindly fill out at the time of your visit. This is not homework!
